Insurance Information
We are committed to working with you to be sure that you understand your financial responsibility for the services provided by Signal Point Hematology Oncology.
Depending upon whether you have insurance, and the extent of your coverage, you may be asked to make a payment at the time of your visit. We suggest that you contact your health insurance provider before your appointment to verify benefits and coverage and to confirm that your Signal Point physician is a participating provider.
Insurance Providers
Signal Point Hematology Oncology is a participating provider with Medicare and most insurance plans. Contact Signal Pont and/or your insurance carrier if you have any questions or to confirm network participation.
Payments
Your co-payment, coinsurance and deductible are due at the time of service. For your convenience Signal Point accepts, cash, personal checks, Visa and Master Card. Please remember to bring your insurance card, Medicare card and or Medicaid card with you to the office visit. If you do not have coverage and have not made financial arrangements in advance, we will ask that you pay in full at the time of your visit.
Health Plan or Managed Care Plan
If you are enrolled in a health plan or managed care plan, your co-payment will be collected before you see the doctor.
Medicaid/Managed Medicaid
If you are enrolled in Managed Medicaid, your co-payment will be collected at the time of your visit. No payment is required for traditional Medicaid. A physician referral is required for Medicaid and Managed Medicaid patients.
Medicare/Managed Medicare
If you are enrolled in traditional Medicare, 20% co-insurance will be collected at the time of your visit. However, if you have a secondary coverage to Medicare, no payment is required.
Out-of-Network Insurance
If Signal Point Hematology Oncology is no included in your insurance plan, we will ask for payment at the time of your visit.
No Insurance.
If you do not have health insurance, we ask that you pay in full at the time of your visit. Please contact our billing department at 513/423-0504 if you have any questions and to set up a payment plan.
PRIMARY INSURANCE CLAIMS
If you have insurance coverage, please understand that this is an agreement between the patient and their insurance company. We will be glad to submit your primary and secondary insurance. If after 60 days of submitting your insurance claims, your insurance carrier fails to pay, the responsibility will be the patients. You are responsible for referral forms from your primary physician if this applies to your insurance policy. If you have problems or concerns, please don't hesitate to contact our office at 513-423-0504.
Due to decreasing reimbursement, Signal Point is making the following changes to our current billing policies, effective March 1, 2010.
- Please bring a valid method of payment with you to your first 2010 appointment. We accept credit/ debit cards (VISA or Master Card only). Please bring a method of payment with you to all appointments.
- Your deductible/co-insurance due will be collected at the time your receive treatment, unless payment arrangements have been made.
- Please bring your current insurance card to all your appointments, so we can verify your insurance status if necessary.
- All patients are required to complete a 2010 registration form.
- Please bring with you a valid state approved identification card.
- All patients will be required to provide the name and phone number of their pharmacy of choice at their 2010 appointment.
- We also would appreciate a 24 hour notice if you find it necessary to cancel. If you are forgetting appointments frequently, we may generate a $25 charge for those appointments you miss without giving us a 24 hour notice.
